Jr umpire game report

This evaluation is only to be completed by junior umpires who have officiated games.   If you are a coach or manager there is a Junior Umpire evaluation form that can be found under the coaches corner on the website. 

All responses are submitted only to the Umpire in Chief, League President, and AAA or AA player agents.  Comments may be viewed by members  of the board of directors.  Comments made in this evaluation may be used to address issues with coaches and managers but will not be directly provided to the coaches or managers.
